IYSA Teams (YSSL and IWSL)

• IYSA Tournament Roster (Players listed in alphabetical order)
• IYSA State Approved Roster
• Player Passes
• Guest Player Form (IYSA on-line form)
• Travel Permit from your State organization (for out of state teams)

US Club Teams (NISL)

• US Club Approved Roster
• Signed IYSA Medical Release & Liability Waiver for each Player (form.pdf)
• US Club Guest Player Form
• NT Form for US Club teams (form.pdf)
